In the generally accepted course of medical practice, this is a variety of pain for which no relief or cure is currently available. If after following our rigorous evaluation and treatment regiment your pain still does not go away, you may be suffering from intractable pain. If this is the case, we will work with you to provide you with the most effective ways of managing your pain condition.

What should I bring to my appointment?

You should bring the following items to your appointment:
•    Your insurance card
•    Physician referral forms if required by insurance
•    A list of current prescriptions and/or over the counter medication you are taking including dose and   frequency

•    Recent X-rays, or appropriate records that you may have

What is the easiest way to reach the nearest Neurology and Pain Management office?

Lafayette is closest to Indianapolis and Merrillville is closest to Gary and the greater Chicagoland area.
